Choices 3 Installation Instructions
It is strongly recommended that an IT professional perform this installation. Simmons will not be responsible for any lost data, corruption, loss of function or any other problems associated with this upgrade or software.
Please perform this installation using the menu and in the order presented.

Step 1
Insert the Choices3 Installation CD in the PC CD-ROM drive.
a. If the program does not automatically start please go to START à RUN and type D:\SLCDMENU.EXE, where D: is your CD drive.
Step 2
On the menu that appears, click the 2nd button “Install Choices3 Software”.
This will install the Choices3 software
Step 3
Follow the prompts to install the Choices 3 Software. Please select the default options. Do not change any paths or options. The sample survey installation does not have to be installed and can be unchecked.
Step 4
Click on the button “Create Unique PC ID”
Step 5
Select Method 1 and click the Close Button (or X) in upper right corner.
Step 6
Open the Choices 3 software, using either the icon on the desktop or thru the Start Menu, and a box with a Unique PC ID will appear.
Step 7
On the menu, click button #4. You will be directed to the activation website. Fill out the form and an email will be sent containing your activation code.
Step 8
Type in your email address and the Unique PC ID copied from step #6
Step 9
When you receive your activation code in an email, cut and paste the code into the activation box and press “Activate”
Step 10
Click the EXIT button on the menu.
